Syml DynaForms Patient+ is a secure digital patient data capture solution to empower and enable patient engagement for quality information collection, while they visit the GP practice. Co-created with GPs and their patients, GP practices can now improve patient participation to support information gathering for better monitoring and precautionary measures.
Patients can complete specific chronic condition monitoring, at-risk assessment or survey questionnaires, and any other customised information needs. All Patient input is available real-time on the clinician’s desktop, saving essential clinical time and improving patient satisfaction.
